  • : It comprises a fixed ratio of 1:6 parts of artemether and lumefantrine respectively.The site of anti-parasitic action of both components is the food vacuole of the malaria parasite, where they are thought to interfere with the conversion of haem, a toxic intermediate produced during heamoglobin breakdown, to the non-toxic haenozoin, malaria pigment. Lumefantrine is thought to interfere with polymerisation process while artemether generates reactive metabolites as a result of the interactions between its peroxide nucleic acid, and protein synthesis within the malaria parasite. Data from in-vitro and in-vivo studies show that it did not induce resistance. The antimalarial cativity of the combination of lumefantrine and artemether in it is greater than that of erther substance alone.

    Composition
    Each tablet contains Artemether20mg,Lumefantrine120mg.

    Indication
    Itis a combination of artemether and lumfentrie, which act as a blood schozincie It is ndicated for the treatment of adultsand children with acute, uncomplicated ifctions due to Plasmodium faliparum or mixed ifectio including P. Facipariumand strains from multi drug resistant areas.
    ltis recomended for use as a standby emergency treatment for taeller to areas where the prasite is rsisant to other drugs.

    Contraindications

    It is contraindicated in:
    -Hypersensitivity to artemether, lumefantrine or to any of the excipients ofit
    -Patients with severe malaria according to WHO definition.
    -First trimester of pregnancy.
    -Patients with a family history of congenital prolongation of the QTc interval or sudden death or with any other clinical conditionknown to prolong the QTc interval such as patients with a historv of symptomatic cardiac arrhythmias, with clinically relevantbradycardia or with severe cardiac disease.
    -Patients with known disturbances of electrolyte imbalance e.g hypokalemia or hypomagnesaemia.
    -Patients taking any drug, which is metabolized by the cytochrome enzyme CYP206 (e.g Hecainde, metaprolol, imipramine,amitriptyline, clomipramine)
    -Patients taking drugs that are known to prolong the QTc interval such as antarrhythics of classes la and II, neuroleptics, andantidepressive agents.

    Adverse effects

    The following adverse effects have been reported; dizziness and fatigue, patients receiving it should not drive or use machine, anorexia, nausea, vomiting, abdominal pain, palpitations, myalgia, sleep disorders, arthralgia, headache and rash.

    In children and adults treated with this combination, the frequency and degree of QTc prolongations were lower compared with other antimalarials. Studies show no indications of cardio toxicity.

    Dosage and Administration
    For Oral Administration
    should be taken with  high fat food or drinks such as milk. Patients should be encourages to resume normal eating as soon as food can be tolerated since this improves absorption of artemether and lumefantrine. On the event of vomiting within 1 hour of administration, a repeat dose should be taken.
    Adults: 1 Tablet for a start and  1 Tablet to be repeated after 8 hours and then 1 tablet to be taken 2 times daily for the next 2 days(Total6 tablets).
